Application Process

All of the following are required for your application to a PGY1 or PGY2 residency to be complete:

Official college transcripts (pharmacy and pre-pharmacy coursework) - these should be in a sealed official envelope
Complete the online Shands HealthCare application
- For PGY-1 resident applicants, please apply for requisition # 14613 (Position title: Resident - Clin Pharm)
- For PGY-2 resident applicants, please apply for requisition #5544 (Position title: Resident - Spec Pharm)
Three (3) letters of reference (using the downloadable forms below; most applicants' references send a letter and this completed form)
Letter of intent (usually one page in length)
Curriculum vitae
